UI updates to template column creation modal

We've redesigned the template column creation modal to better organize our different data validations options.

To provide more clarity, we've renamed the "Required" validation to "Must be mapped". We've also renamed the "Column must exist" validation to "All cells must be filled". A short description has been added to each validation option to provide more context into how each option affects the end user experience.
